Must have FL or Compact license on file at time of submission. they will need to have a bachelors degree. Special Notes: Candidatesshould have experience working in an acute care hospital with extensiveexperience with discharge planning, Cerner, Care Port, able to completepsychosocial assessments, have knowledge on advanced directives and socialdeterminants of health.
